من دکتر رضا فرجی، استادیار و عضو هیأت علمی دانشگاه علوم پزشکی بابل، و متخصص ارتودنسی هستم. تا کنون در کنار تدریس در دانشگاه، و فعالیت در زمینه های آموزشی، علمی-پژوهشی، تحقیقاتی، … خدمات متعدد پیشگیری، تشخیصی و درمانی را به عنوان یک ارتودنتیست ارائه کرده ام. وظیفه ی من مانند دیگر متخصصان این شاخه از دندانپزشکی اصلاح ناهنجاری های صورت و دندان ها و بهبود عملکرد آنها جهت سلامتی فرد (دندان، لثه، فک) و بخشدین لبخندی زیبا به چهره ی آنها می باشد. معمولا این گونه مشکلات و ناهنجاری ها از سنین پایین آغاز می شوند، و در صورت عدم درمان به موقع می توانند شدت یافته و روی جویدن و صحبت کردن فرد، سلامت لثه ها، سلامت دندان ها، … تأثیر بگذارند، در مورد ناهنجاری های فکی نیز اگر به موقع درمان نشوند می توانند موجب دردهای ناحیه ی گردن، شانه ها و پشت شوند. شما می توانید با انجام ارتودنسی در زمان مناسب به سلامت دهان و دندان های خود و همینطور تصحیح عملکرد آنها کمک بکنید. باید یادآور شد که بهترین زمان مناسب جهت انجام ارتودنسی در مواردی که فرد با مشکلات اسکلتی (ناهنجاری های فک) مواجه هست، قبل از بلوغ می باشد چرا که با رشد کودکان و نوجوانان می توان این مشکلات را برطرف و اصلاح کرد، اگر ناهنجاری شدید باشد و فرد جهت درمان به موقع اقدام نکند ممکن است نیاز به جراحی فک پیدا کند. اما در مورد مشکلات دندانی محدودیت سنی وجود ندارد، هر چند در این مورد هم هر چه سن فرد پایین تر باشد زودتر درمان جواب خواهد داد، اما با این حال بزرگسالان نیز می توانند از خدمات ارتودنسی جهت رفع و اصلاح ناهنجاری های دندانی خود بهره مند شوند، تنها تفاوت آنها با کودکان و نوجوانان در طولانی تر بودن پروسه ی درمان می باشد، باز هم نیاز به یادآوری است که تمامی مواردی که اشاره شد بستگی به نوع و شدت ناهنجاری دارد. صحبت از ناهنجاری های فک و دندان ها شد، که در این قسمت می توانم به شرح مهم ترین و شایع ترین این ناهنجاری ها بپردازم:
– اور بایت (Over bite): پوشانده شدن دندان های پایین توسط دندان های بالا
– آندر بایت (Under bite): به وضعیتی گفته می شود که دندان های ردیف پایین بسیار جلوتر از دندان های ردیف بالا قرار می گیرند، به آن ظاهر بولداگ هم گفته می شود.
– کراس بایت (Cross bite): در این ناهنجاری معمولا یک تا چند دندان از ردیف بالا به درستی روی دندان های مقابل خود روی ردیف پایین قرار نمی گیرند.
– اوپن بایت (Open bite): به شرایطی گفته می شود که با وجود قرار گرفتن دندان های فک بالا و پایین روی هم، باز هم بین آنها فاصله وجود دارد.
– ریورس بایت (Reverse bite): هنگام درگیری دندان های ردیف بالا و پایین، دندان های ردیف بالا در داخل دندان های ردیف پایین فرو می روند.
– اسپیسینگ (Spacing): وجود فاصله بین دندان ها، که می تواند به خاطر عدم رشد یک دندان، رشد ناقص دندان ها و یا از دست دادن یک دندان ایجاد شود.
– کراودینگ (Crowding): بر عکس اسپیسینگ، در این مورد ما با شلوغی و به هم ریختگی دندان ها مواجه هستیم.
– دندان نهفته (Impacted teeth): که معمولا یا به صورت کامل درون لثه قرار دارند یا تنها بخشی از آنها از لثه بیرون است.
– اور جت (Over jet): جلوزدگی بیش از اندازه ی دندان های جلویی فک بالا
– جلو بودن یکی از فک ها: که می تواند بر اثر عوامل ژنتیکی، و عادت های دوران کودکی مانند مکیدن انگشت ها ایجاد شود.
– عقب بودن یکی از فک ها
– انحراف فک: زمانی که خط میانی دندان های بالا و پایین در یک راستا نباشد، عارضه ی انحراف فک وجود دارد.
– بزرگ بودن فک
– کوچک بودن فک

آیا ارتودنسی یک فک امکان پذیر است؟
برخی بیماران با هدف کاهش هزینه ارتودنسی و یا با تصور مشکل نامرتبی کمتر دندان ها در یک فک، این سوال برایشان پیش می آید؛
ولی متاسفانه در اغلب موارد امکان درمان ارتودنسی یک فک وجود ندارد.
درمان ارتودنسی فقط برای ردیف نمودن دندان های یک فک نیست. در واقع باید هر دو فک با یکدیگر همخوانی داشته باشند،

اپن بایت چیست؟
اپن بایت نوعی نامرتبی دندان هاست که در نتیجه آن دندان های فک بالا و پایین روی هم قرار نمی گیرند.
اپن بایت به دلالیل ژنتیکی یا عادت مکیدن انگشت یا استفاده طولانی مدت از پستانک ایجاد می شود.

خارج شدن سیم ارتودنسی
برخی از سیم های ارتودنسی به ویژه در مراحل اولیه درمان بسیار انعطاف پذیر هستند.
این امکان وجود دارد که از روی براکت ارتودنسی بلغزند و خارج شوند
با رعایت رژیم غذایی توصیه شده توسط متخصص ارتودنسی، احتمال بروز این مشکل به حداقل می رسد
اگر انتهای سیم از داخل دستگاه خارج شد سعی کنید با یک پنس یا موچین، سیم را به محل قبلی خود بر گردانید
در صورت ضرورت، یک قطعه موم ارتودنسی بر روی سیم قرار داده و با ارتودنتیست خود تماس بگیرید
